DMV Confidentiality Request

The request for confidentiality of home address (INV 32) Forms are no longer available for pickup at local DMV’s.

They are available only through the qualifying agencies. The DMV Confidentiality of Home Address covers LASD retired peace officers holding a valid California drivers license.

LASD retired peace officers can request the (INV 32) Form by contacting:
